Customisable FARYDAK tracker calendar

At treatment initiation, this customisable, easy-to-use dosing calendar allows you to enter the patient’s name and select a therapy cycle and start date for FARYDAK. With just a few clicks, you’ll be able to print or email a personalised medicine tracker calendar for your appropriate patient.

If any changes are made to a patient's dosing schedule, a new medicine tracker calendar will need to be created and given to the patient. The patient should discontinue use of any existing medicine tracker calendars.

For patients >75 years of age, depending on the patient’s general condition and concomitant diseases, an adjustment of the starting doses or schedule of the components of the combination regimen may be considered.

FARYDAK may be started at a dose of 15 mg and, if tolerated in the first cycle, escalated to 20 mg in the second cycle
Bortezomib may be started at 1.3 mg/m² once weekly on days 1 and 8, and dexamethasone at 20 mg on days 1 and 8

Medicine tracker calendar help

4 Choose which month the treatment will start. Use right and left arrows to choose the appropriate month. Then click a start date to generate a personalised calendar with the chosen treatment regimen.

5 Once generated, use the buttons on the bottom of the calendar to email a copy of the tracker, view and print the tracker, or start over (clears all previous entries).
